Terms and Conditions

This agreement shall be effective on the date of acceptance of the Benefit participant's application for registration in the JB program and payment of the registration fees.

 

Neither JB, nor its administrator is an insurance company, agent, or agency in any state, including Texas, Oklahoma, or California. Medical, chiropractic, dental, and other member access is available through various "access providers", not Just Benefits, and provide members with access to those health care providers willing to provide low, member pricing to members. All matters between the members and health care providers are handled directly between the members and the health care providers, without participation by JB.

 

The JB program is not an insurance policy. No portion of any provider's fee or cost will be reimbursed or otherwise paid by JB. The member is solely responsible for payment of all provider fees and costs. JB's only obligation under this agreement is to administer registration of the Benefit participant with participating providers. JB discounts cannot be utilized in conjunction with any other discount program.

 

It is the responsibility of the Benefit participant to call JB to verify that a provider is a current participant in the program. Benefit participant will always be responsible for payment for the services provided as well as related expenses. Benefits offered by JB may vary from state to state, and service provider's are subject to change at any time without notice.

Membership is currently Available in these States.

 

1. 1. JB is not a discount buying service organization. The goods and services made available to members may be available from time to time in member's locale from the same or similar service providers and at the same or lower cost. Just Benefits strives at all times, but does not guarantee, to provide the lowest cost to its members from the service providers. The actual savings you derive may vary, depending upon your location and the specific services or products purchased. Savings will be derived from a discount from the provider's published fees, as the same may change from time to time. All fees listed or quoted by JB are based upon information, which they believe to be accurate. However, all fees and prices are subject to change at any time without notice. In the event the services or products are being offered to the general public by the selected provider at a price less than the JB price, Benefit participants should be charged the lower price.
    

2. All member service providers are fully licensed in their applicable states of service to provide the specified services to members. Just Benefits is not an insurer nor does it provide payment or indemnity to members nor is it licensed to provide nor does it provide, life insurance, medical insurance, dental insurance, or any related form of preferred provider service. 

3. All services available to members are provided by third parties, not by JB. Members agree to look exclusively to our service providers and that Just Benefits is neither responsible for nor liable for the acts and omissions of any service provider. Just Benefits continues to seek the most experienced and capable service providers at the lowest member cost and will provide member assistance in processing inquiries, complaints, and service requests of member with regards to service providers.
    

4. The family membership entitles all eligible family household members to all benefits in the plan they purchased except two benefits.

5. By retaining my membership longer than 30 days, I do hereby certify and attest that my representative did at no time during the presentation refer to Just Benefits as an insurance company, and did at no time represent the Membership plan as health insurance. I fully understand that I am purchasing a membership with Just Benefits and not health insurance.

6. JB dues and memberships are on a month-to-month basis with the payments due in advance. JB reserves the right to terminate any membership or deny eligibility in the program for lack of payment to JB. Insufficient fund notices on bank drafts or denial by the member's credit card company for payments on the recurring program fee is deemed evidence of non-payment. There will be a ten-dollar ($10.00) charge to the member for all denials of payment.
    

7. Except as expressly provided herein, JB's maximum liability hereunder shall be limited to a refund of all fees paid to JB in the thirty (30) day period preceding the notice of any claim or demand. If you decide to cancel this agreement due to a modification of the JB program within thirty (30) days of such modification, the joint maximum liability of JB shall be limited to a refund of the monthly fee paid for the period subsequent to the modification.

 

This agreement constitutes the entire agreement between Benefit participant and JB. THERE ARE NO WARRANTIES; EXpRESS OR IMPLIED OTHER THAN EXPRESSLY STATED HEREIN. Benefit participant hereby waives any claim he or she may have against JB attributable to ministerial or typographical errors. This agreement may only be amended by JB in writing. JB may, if it deems necessary, assign its duties and responsibilities hereunder to third parties, and shall be relieved of any further liability hereunder.

This agreement shall be governed and construed in accordance with the laws of the state of Texas. Any dispute arising from or relating to this agreement shall be resolved by binding, non-appealable private arbitration, conducted in accordance with the rules of the American Arbitration Association. Exclusive venue for such arbitration shall be in Houston, Texas, unless otherwise designated by JB or its successors. These provisions shall survive termination of participation in the JB program.
